Required Health Records at Certain Hospitals

Per the Student Health Records Department, the following hospitals require you to present them with full records:

1. Jamaica Hospital

2. Flushing Hospital

3. Kings County Hospital

4. Woodhull Hospital

5. Mountainside Hospital

6. Richmond University Medical. Center (RUMC)

7.  Manhattan Psychiatric Center

If you are going to be starting at one of the above hospitals ANYTIME during your clinical years, you need to contact Susan Conway as soon as your rotation has been scheduled.  Students who wait until a couple of days before the start date are not guaranteed that they will get their records and will be restricted from starting.   The Student Health Records Department does NOT send your records to the hospital. It is a HIPPA violation for anyone to receive your health documents except for the Occupational Health Department.  The Student Health Records Department send students a pdf copy of their file. You will need to copy and SAVE the file for further use. You will present the paper copy to the Occupational Health unit at the hospital you are rotating.  Students have been notified numerous times that they should always have their original health records in their possession.  Some hospitals may request to see the actual lab copies, and it your responsibility to be able to access them.  Fourth year independent rotations will require the student to present their  own original health records to the Occupational Health Department.  The Student Health Records Department does not fill out hospitals health forms.

While this list should hopefully be comprehensive, it can change at times (and we may not be notified).
